Employee Access to Remote Work Agreement

Outside Activities Report (OAR)

All faculty, regardless of appointment, all academic staff with 50% or greater appointment, and all individuals listed as participants on human subject protocols or on federal grants are required to fill out an annual Outside Activities Report (OAR) and update whenever new outside activities are undertaken.

Individuals must submit the Outside Activities Report even if they have no outside financial interests in their field of work at the University.

Annual reports must be submitted by April 30.

Faculty Workload Policy

This policy is intended to provide the department with guidelines for reasonable average workload assignments for each faculty member. The workload guidelines are based on the effort required for conducting formal classes, and other faculty activities are credited in terms of instructional equivalencies. In addition to the workload expectation, and regardless of total workload, each faculty member is expected to meet a minimum instructional workload. There is also a minimum classroom-instruction expectation to ensure sufficient faculty involvement in the Department’s curricula.

ERB UPS Form

The NEEP Dept has a UPS account and packages can be shipped from the ERB loading dock.

Please complete the ERB UPS Form, attach it to your package and place the package on a table in Mitch Lange’s office (located in the ERB loading dock).

Keys

KeyLime – New Key Request

KeyLime is the CoE Key Management system for all key requests. Log-in using your NetID to request keys and view current building access.

Travel

W-Madison Travel Rules

Please reference the UW-Madison Travel Rules in advance of booking travel. All rules and regulations must be followed in order to be reimbursed for your travel.

Travel Request Form

The NEEP Dept Travel Request Form must be completed in advance of booking travel. Completed forms should be emailed to travelrequest@ep.wisc.edu.

You will NOT receive a reply confirming the receipt of your travel request form. Proceed with making travel arrangements for the payment of registration fee, airfare, or lodging reservations after submitting your completed form.

Concur Travel Booking

Fox World Travel is the University’s contracted travel agency for individual travel. Concur is the online booking portal administered by Fox World Travel.

Driver Authorization

Any employee who will be operating a vehicle on UW business must be an authorized driver prior to traveling. This includes rental vehicles, fleet vehicles and personally owned vehicles.

No vehicle-related expenses will be paid or reimbursed for unauthorized drivers.

NEEP Department Cost Comparison Guidelines

The NEEP Department Cost Comparison Sheet should be used when travelers save overall travel costs due to plans or itinerary.

If personal travel is combined with business travel, an airfare cost comparison from Fox World Travel (Concur) is required. The University will not incur additional expense for personal travel.

The Assessment, Evaluation, Feedback, & Intervention System (AEFIS) is a cloud-based assessment management system that facilitates the collection and application of learning assessment data.

The Center for Teaching, Learning & Mentoring (CTLM) supports and provides resources for instructors for continued growth in teaching.

The Office of Youth Protection (OYP) is a support unit to campus with responsibility for overseeing and enforcing the university’s Youth Protection Policy (UW-1045).

Project Assistant Grader/Reader Application

The NEEP Department Chair and faculty determine which instructors/courses may have an open Grader position. Usually only a limited number of positions are available.

The purpose of this application is to standardize how information is collected from applicants; to provide a central repository the Department Chair and faculty can access for review; and to provide data tables related to Course support.
